The j2 Global survey found that although small business owners still rely most heavily on more established marketing methods, such as email marketing and online search, social media is fast becoming ?the great equalizer,? putting a small business on an equal footing with bigger competitors and enabling them to engage with customers, spread the word about their businesses and more

More than one in three small businesses surveyed by j2 Global?(39 percent) say that one-quarter of their marketing strategy is now focused on social media, and 26 percent say social media accounts for half or more of their marketing strategy. In other words, nearly two-thirds of small businesses are making social media a major part of their marketing platform. Are you?

If not, it?s time to get in on the action. Where are small businesses seeing the most success with social media? Here?s what j2 found:

  • Facebook is the most valuable social media platform for 37 percent of respondents. Next in line were LinkedIn (24 percent) and Google+ (18 percent).
  • Which social media platform small businesses prefer varies by industry. For instance, restaurants use Facebook the most, consultants use LinkedIn the most, medical professionals prefer Google+ and eCommerce businesses rely on Twitter. Somewhat surprisingly, YouTube was the preferred platform for high-tech firms.
  • Pinterest, the newest social media site out there, was rated most valuable by just 1 percent of respondents. I?m willing to wager that will change soon, as Pinterest is proving to have value for retail, restaurant and home d?cor companies.

Police: Woman tried to break into Ohio jail

(AP) ? Police in southwest Ohio are perplexed about why a woman tried to sneak into a county jail before telling authorities to arrest her.

Deputies with the Butler County Sheriff's Office arrested 36-year-old Tiffany R. Hurd on Sunday morning after she was caught trying to climb over a fence into Butler County Jail in Hamilton, near Cincinnati. It happened after jail staff leaving a late-night shift told Hurd to leave the property, but she told them to arrest her.

"She was repeatedly told to stop," Sgt. Monte Mayer said. "They couldn't talk her out of it."

Deputies asked Hurd to leave numerous times, but she refused and attempted to climb the fence again. That's when police took action.

"She got her wish," Mayer said. "It wasn't in a traditional manner."

Deputies say Hurd appeared to be intoxicated. In a statement about the incident, Sheriff Richard Jones said Hurd's actions caught him by surprise.

"I know the economy is bad right now, but I didn't think it was so bad that someone would actually try to break IN to jail," he said.

Mayer said his colleagues had never heard of such a situation.

"To the best of our knowledge, we have never heard of someone trying to break into a secure area of the jail compound, hoping to get into jail," he said. "That's a first."

Hurd was arrested on misdemeanor charges of criminal trespassing and disorderly conduct. Bond was set at $2,500 during an arraignment Monday.

Hurd remains at the county jail. She has another court appearance scheduled Aug. 9. An attorney listed for Hurd did not immediately return a message Monday seeking comment.

UPDATE 1-Enbridge races to clean up Wisconsin oil spill, restart line

GRAND MARSH, Wis., July 29 (Reuters) - Canada's Enbridge Inc. on Sunday raced to repair a major pipeline that spilled more than 1,000 barrels of oil in a Wisconsin field, provoking fresh ire from Washington over the latest in a series of leaks.

The spill on Friday -- almost two years to the day after a ruptured Enbridge line fouled part of the Kalamazoo River in Michigan -- has forced the closure of a major conduit for Canadian light crude shipments to U.S. refiners and threatens further damage the reputation of a company that launched a more than $3 billion expansion program just two months ago.

Enbridge said it intended to begin repairs to Line 14 late on Saturday after making 'excellent progress' in clean-up, allowing for visual inspection of the line. But it still did not know what had caused the incident and provided no estimate on when the 318,000 barrels-per-day Line 14 would resume service.

An official with the U.S. Pipeline and Hazardous Materials Safety Administration (PHMSA) said two inspectors were at the site on Sunday, and that all of the pooled oil had been cleaned up.

'The line has been uncovered to begin removing the failed section and send it to a metallurgical lab for examination,' PHMSA spokesman Damon Hill said.

Officials from the U.S. Environmental Protection Agency (EPA) and the Wisconsin Department of Natural Resources are also on site, Enbridge said in a statement.

An image of the area posted on Enbridge's website showed a patch of damp, blackened earth near a stand of trees about one-third the size of a football field. It found some oil on two small farm ponds, but said they did not connect to moving waterways and that drinking wells did not seem to be affected.

Although the spill appeared to be relatively small and quickly contained, it comes at a delicate time for Enbridge, which suffered another leak in Alberta, Canada, a month ago and endured a scathing report from U.S. safety regulators over its handling of the Michigan incident in 2010, with employees likened to the 'Keystone Kops' for their bungled response.

'Enbridge is fast becoming to the Midwest what BP was to the Gulf of Mexico, posing troubling risks to the environment,' U.S. Representative Ed Markey, the top Democrat on the Natural Resources Committee, said in a statement.

'The company must be forthcoming about this entire incident, and deserves a top-to-bottom review of their safety culture, procedures and standards,' said Markey, an outspoken critic of increasing imports of Canada's heavy oil sands crude.

Just two months ago, Enbridge kicked off one of the most sweeping expansions in its history, announcing a multibillion-dollar series of projects aimed at moving western Canada and North Dakota oil to Eastern refineries and eliminating costly bottlenecks in the U.S. Midwest.

Line 14 is a 24-inch diameter pipe that was installed in 1998, making it a relatively new line. Enbridge said it had been inspected twice in the past five years.

TWO LANDOWNERS, ONE HOUSE 'COVERED'

In most cases, smaller pipeline leaks can be repaired quickly, although regulators may require significant work if they find any cause for alarm. Following the leak in Michigan two years ago -- which spilled roughly 15 times more oil than the Wisconsin leak if initial estimates of the Friday incident prove correct -- one line was shut for more than two months.

Enbridge said two landowners had been affected and that one family had been relocated for their safety and comfort, but that most of the spill was restricted to the pipeline right-of-way. It kept its estimate of the spill at around 1,200 barrels -- about as much as would fit in six very large oil tanker trucks.

'The house right next to where the pipeline broke got covered with oil,' said Patrick Swadish, who lives about a mile northwest of the spill site in a rural area of mostly farmland, some 80 miles north of the college town of Madison.

Oil trucks, Enbridge vehicles and about a dozen crews were working in the area, which had been cordoned off by sheriff deputies. Local law enforcement officials said they had been told it may take up to 30 days to clean the area.

Enbridge also said it had briefly shut down two larger adjacent lines -- the 400,000 bpd Line 61 and the 670,000 bpd Line 6A -- but both were pumping again within a day. Together with Line 14, they form the backbone of Lakehead, a 2.5 million bpd network that is the main route for Canadian exports.

Another line, the 180,000 bpd Line 13, which carries diluents from Chicago to Edmonton, Alberta, would be restarted once it was confirmed the release had not had an impact on it, it said.

PREVIOUS SPILLS

Just weeks ago, the U.S. National Transportation Safety Board blasted Enbridge's handling of the July 2010 rupture of its Line 6B near Marshall, Michigan, which led to more than 20,000 barrels of crude leaking into the Kalamazoo River.

The NTSB said it found a complete breakdown of company safety measures, and that Enbridge employees performed like 'Keystone Kops' trying to contain it. The rupture went undetected for 17 hours.

U.S. pipeline regulators fined it $3.7 million for the spill, their largest ever penalty.

The incidents, plus the most recent spill in Alberta, have caused furor just as the company seeks approval for its C$6 billion Northern Gateway pipeline to Canada's West Coast amid staunch opposition from environmental groups and native communities that warn against oil spills.

There?s long been debate over fertility drugs and if they could increase a woman?s risk for breast cancer. A new study found that women who used fertility drugs and got pregnant had a small increased risk of breast cancer.

Experts say it could be due to elevated production of ovarian hormones that alter breast tissue.?

Sandra Mueller of Sioux City attributes her bout with breast cancer to fertility treatments.

She?s like many moms: she?s busy and life is sometimes hectic, but Sandra?s greatest joy is spending time with her four year old son, Logan.

Still there was a time when Sandra and her husband, Rex, weren?t sure if they?d get to experience parenthood. They wanted a child so badly, they went through four grueling years of fertility treatments.

?We just knew that if we wanted this to happen ? at the time, we thought ?we have to do this?, because this is the only way it?s going to happen. It?s a very emotional process and the desire to be a parent is so great that you want to try and do what you can to make it happen,? Sandra says.

Sandra chose to have a bi?lateral mastectomy. And, because the cancer was found so early, she didn?t need chemotherapy or radiation. But, Sandra suspects it was those fertility treatments that were behind the breast cancer.

?There was no other connection that I could make and I had done some research on my own. We have history of cancer in the family, but we?ve never had anybody with breast cancer,? she says.

Now a breast cancer survivor, it?s clear Sandra and Rex have had their prayers answered. They became parents in 2007, when they adopted their son, Logan.

?He?s just been the center of our world ever since and he is everything and then some and we were meant to be ? we were all meant to be together,? Sandra comments.

And does she regret the trials that brought her here? No.So, her advice to other women, maybe isn?t surprising.

?I would never tell a woman: ?don?t do fertility treatments,? I would never tell somebody that ? I would just want to say, ?ask questions ? find out about what your body?s going to be going through,? she says.

And, through it all, Sandra?s faith helped her believe this day would come. And boy, was it worth the wait.

?Being a mom has just been the most wonderful blessing and I am so grateful that I have been able to experience it.?

JERUSALEM (AP) ? Mitt Romney says that if he paid more taxes than were required, he wouldn't be qualified to be president.

The U.S. presidential candidate tells ABC News that American voters expect candidates to pay "only what the tax code requires." He says he hasn't calculated what percentage of his income was paid in taxes but says that he's been audited.

Romney's vast personal wealth has been an issue in the campaign.

Democrats have criticized him for paying only 13.9 percent of his income in taxes during the one year his taxes have been made public.

Romney says he will release an additional year of returns before Election Day. That has left some questioning whether Romney pays a smaller share than other Americans.

The Challenge of Healthy Eating ? The What's for Lunch Project

that replacing two meals per day with Special K products helps consumers can result
shedding up to six pounds in the short span of two weeks. Special K?s meal replacement
options are vast?and their touted benefits are enticing. There are ten varieties of
breakfast cereals, snack bars, granola bars, and meal bars, protein shakes, protein water,
and even crackers and chips, all the designed to be low in calories and high in diet-
payoff.

And with product flavors like ?Chocolatey Delight,? ?Raspberry Cheesecake,?
and ?Brown Sugar Cinnamon,? the Special K Challenge seems like a pretty sweet deal.

But before you dig into that first spoonful of cereal, take a moment to consider the costs
of a low-calorie diet based on highly-processed foods. Consider that there?s more to lose
on the Special K diet than just six pounds.

For evidence, start with the most important meal of the day?and compare Special K?s
Fruit and Yogurt cereal to Post?s ubiquitous morning staple, Shredded Wheat.

With a half cup of skim milk added, a serving of Special K Fruit and Yogurt weighs in
at 160 calories, with 10 grams of sugar, 3 grams of fiber, and 2 grams of protein. It also
boasts an ingredients list that?s over 30 items long. Additionally, that ingredients list
includes seven types of sweeteners: sugar, corn syrup, brown sugar syrup, brown sugar,
confectioner?s glaze, honey, and molasses.

A one-cup serving of Shredded Wheat with the same amount of milk has a higher caloric
price tag, at 210. But for those extra calories, breakfasters get 6 grams of both protein and
fiber?and no sugar at all. What?s more? Shredded Wheat has a single ingredient: whole
grain wheat.

Special K?s cereal has less calories?but it?s also nutritionally poorer, with more of the
excess sugar that can contribute to diabetes and less of the satisfying protein and fiber to
fill a hungry stomach. Most Special K products are similarly guilty. The French Vanilla
protein shake packs 18 grams of sugar (nearly as much at a pack of Reese?s peanut butter
cups) and the Chocolate Caramel meal bar has 15.

Still, the promise of losing 6 pounds in two weeks coerces many consumers to purchase
and eat the empty, sugary calories provided by the Challenge-approved products.

And if two of your meals each day are bowls of cereal at only 160 calories each, losing
weight is almost an inevitability. But the Special K Challenge comes at a cost. In order
to achieve the average participant?s weight loss of 4.8 pounds, those who partake in the

Challenge sacrifice adequate nutrition from whole foods.

But it seems many Americans?especially those concerned with weight?believe that the
number of calories is the only nutrition statistic that matters.

This is the attitude behind the Speical K Challenge. It?s the attitude behind brands like
Lean Cuisine, and diet programs made entirely of frozen, processed food, like Jenny
Craig. And this attitude will not cure or counter the epidemics of obesity and diabetes in
this country.

Even worse, this attitude has already trickled into the regulation of products sold in
public school lunches. Many states have prohibited the sale of soda in cafeterias. But if
they?re only replaced by diet drinks with unnatural, artificial sweeteners, has the selection
really improved? Replacing soda with diet drinks is not a solution. Simply lowering
calories cannot be a solution.

Americans? relationship with food, with obesity, and with diabetes will change?only
if both consumers and manufacturers start to value caloric quality over caloric quantity.
Our nation will become healthier only if we Challenge ourselves eat better, instead of just
eating less.

New cancer drugs: Fitter, happier, more productive? Or not ...

It?s known as ?targeted? therapy, and it?s the holy grail of cancer research these days. If you listen to its most vocal proponents, it?s the path towards ?personalized medicine? that improves survival with much lower toxicity, in which, instead of using the hammer that is chemotherapy, precisely targets specific molecular abnormalities that drive cancer growth. With the advent of the revolution in genomics that has transformed cancer research over the last decade, including the petabytes of sequence and gene expression data that pour out of universities and research institutes, the promise of one day being able to a patient?s tumor, determining the specific derangements in its genome, gene expression, and protein expression that drive its uncontrolled proliferation, and finding drugs to target these abnormalities seems more tantalizingly close than ever. Indeed, it seems so close that even dubious practitioners, such as Stanislaw Burzynski, have jumped on the bandwagon, co-opting the terms used by real oncologists and real cancer researchers to sell ?personalized gene-targeted cancer therapy,? which in their hands are really no more than a parody of efforts to synthesize the enormous quantity of genomic data each patient?s tumor possesses and figure out how best to take advantage of it, a ?personalized genomic therapy for dummies,? if you will.

That?s not to say that there aren?t roadblocks to realizing this vision. The problems to be overcome are substantial, and I?ve discussed them multiple times before. For example, just a couple of weeks ago I discussed an example of just what it takes to apply these new genomic techniques to an individual patient. The resources required are staggering, and, more problematic, there often aren?t any single ?magic bullet? molecular pathways identified that can be targeted with existing drugs. The case I discussed was a fortunate man indeed in that such a pathway was identified, but most tumors are driven by many derangements in growth control, metabolism, migration, and the other hallmarks of malignancy described by Robert Weinberg. Worse, in many cases we don?t even have drugs that can attack many of the abnormalities that drive cancer progression. Then there?s the issue of tumor heterogeneity, which comes about because cancer is as good example of a disease as I can think of in which evolution due to natural selection results in incredible differences in the cancer cells in one part of the tumor compared to other parts of the tumor or in the tumor metastases. A ?targeted? therapy that targets the genetic abnormalities in one part of the cancer might well fail to target the genetic abnormalities driving another part of the tumor.

These, and many other reasons, are why we haven?t ?cured cancer? yet.

Of course, the main promise of the new targeted therapies, besides more precisely targeting the actual genetic abnormalities that drive the growth of specific tumors, is decreased toxicity. Whereas cytotoxic chemotherapy is like a blunderbuss that pummels and poisons cells, both normal and cancerous (it just poisons cancer cells more, which is why it works), ?targeted? agents are frequently portrayed as ?smart bombs? that attack only the cancer cells and produce little, if any, collateral damage. But is that true? There?s a whole lot of hype, but thus far the reality has been disappointing. If a new study just published online in the Journal of Clinical Oncology by a group at the Princess Margaret Hospital and University in Toronto, the answer is ?probably not.? The study?s title pretty much sums up the findings: The Price We Pay for Progress: A Meta-Analysis of Harms of Newly Approved Anticancer Drugs. However, as you will see, the way the results of the study (Niraula et al) are being reported are not exactly in line with what the study found. In other words, this study finds something that seems unexpected, part of which is a bit unexpected, most of which is not.

A bit of background is in order, and the introduction of the paper is a good place to start:

Although virtually all new drugs are tested in early-phase clinical trials designed to evaluate toxicity and tolerance, these trials are small and can detect only common toxicities. Most large RCTs are primarily not designed to detect differences in quality of life or toxicity of treatment between the study arms and are usually inadequately powered to do so. Furthermore, some anticancer drugs are approved on the basis of smaller, nonrandomized, unblinded clinical trials5 in which detection of toxicity can be severely impaired. Rare but potentially serious toxicities may not be detected in RCTs owing to a selection of patients with high performance status and minimal comorbidity or because of relatively short follow-up. The quality of reporting toxicity in RCTs has been criticized following evidence that some toxicities may be detected but inadequately reported.3,6 Increased treatment-related mortality associatedwith bevacizumab,7 increased cardiovascular morbidity of aromatase inhibitors,8,9 and increased risk of cardiopulmonary arrest with cetuximab10 were all either undetected or unreported at the time of drug approval but were described postmarketing. Therefore, efforts to monitor for toxicities after completing pivotal phase III RCTs are essential.11 The US Food and Drug Administration has encouraged a systematic approach to the collection and reporting of toxicity.12 Until such measures are implemented, there will continue to be concerns about failure of detection and reporting of serious and fatal toxicities.

In essence, the approval process for cancer drugs suffer from the same limitations as the approval process for all drugs does. The phase III studies used to support approval, although large, are not large enough to identify uncommon complications and toxicities or to do a fine analysis of differences in quality of life. Unfortunately, these sorts of issues are often not identified until approved drugs go into wide usage. Post marketing surveillance studies are useful for this purpose, but it?s a bit hit or miss whether such studies are done. Because anticancer agents, even targeted therapies, are directed at critical signaling pathways that control cell replication, death, mobility, and other key processes common in all cells, even targeted therapies are likely to produce at least some toxicity. Basically, all drug approval is a delicate balancing act between doing enough studies with enough patients to ensure that the drug tested is safe and effective versus cost and the time it takes to do the necessary studies.

What the authors hypothesized is that the experimental arms of studies used to support the approval of new agents would be associated with a higher rate of significant toxicity. The specific toxicities studied included treatment-related deaths, any toxicity severe enough that treatment had to be halted, and grade 3 and 4 adverse events (AEs). Grade 3 AEs include ?symptoms causing inability to perform usual social and functional activities,? while grade 4 AEs are potentially life-threatening and result in ?symptoms causing inability to perform basic self-care functions or medical or operative intervention indicated to prevent permanent impairment, persistent disability, or death.? To test their hypothesis, the authors identified 38 RCTs used to support approval of a drug for a cancer indication between January 2000 and December 2010, specifically the phase III randomized clinical trials used to support FDA approval of these drugs. A meta-analysis was performed, and correlations were made between the odds ratios for the three primary endpoints (treatment-related deaths, AEs resulting in treatment discontinuation, and grade 3/4 AEs) associated with the new drug and hazard ratios for overall survival (OS) and progression-free survival (PFS) associated with these drugs.

What the authors found can be boiled down fairly easily. Compared with control groups, the odds ratio (OR) for treatment-associated toxic deaths was elevated (OR, 1.40; 95% CI, 1.15 to 1.70; P peripheral neuropathy is a common complication of taxanes, so much so that many women I?ve operated on who underwent such therapy complain about numbness in their fingers, and one of the main drugs included in the meta-analysis was docetaxel (trade name Taxotere); so I?m not terribly surprised that this toxicity would come to the fore.

There are a number of other issues that were brought up with respect to this trial as well. The results I?ve just described were the overall results. The authors had several pre-planned subgroup analyses, and the results were a mixture of the aforementioned surprising and unsurprising results. The first surprising result is that the authors compared ? targeted drugs directed against a specific molecular target (specifically, the target population to be treated was selected by presence of a biomarker) versus drugs that are less specific (unselected target population, not based on presence of a biomarker). In this classification, docetaxel would be less specific because it is a general microtubule inhibitor that interferes with cell division. Its use is thus not chosen on the basis of an existing biomarker. In contrast, drugs like Tamoxifen or aromatase inhibitors would be considered ?targeted? because they target estrogen signaling and are therefore only used in women whose cancers make the estrogen receptor. Similarly Herceptin would be considered a targeted agent because it targets HER2 and is only used in women whose cancers make this particular oncogene protein product.

The findings were a bit unexpected in that, targeted therapies had a higher OR for the three end points than the ?less targeted? agents. However, I have a bit of a problem with this. Here?s a table describing the OR of the various endpoints for these two groups, with the 95% confidence intervals (CIs) in parentheses below each OR:

Notice that, for the specific targeted agents, the ORs are not statistically significantly different than 1.0 for toxic deaths and treatment discontinuation, but for the less specific agents, the ORs are statistically significantly greater than 1.0 for two out of the three endpoints. So, while it?s true that the risk of grade 3/4 AEs is higher for the targeted agents, the ORs for everything else are not. Personally, I?d call this a wash, or even potentially an indication that the less targeted therapies produce more AEs, as one would expect.

The next thing that the authors did that seemed to get lost in the news reports discussing this study. For instance:

The study showed the newer cancer drugs caused significantly more side effects, and more treatment-related deaths, than their older counterparts. ?You?ve got to consider both efficacy and toxicity in the picture,? said Susan Ellenberg, who has studied drug side effects at the University of Pennsylvania?s Perelman School of Medicine in Philadelphia.

She said the analysis wasn?t so surprising, and isn?t ?a big alarm.?

While it is correct that this isn?t a ?big alarm,? reporting this study in a blanket fashion as showing these newer drugs as causing significantly more side effects is simplifying the results of this study way too much. The reason, I would posit, comes from another pre-planned subgroup analysis that the authors did:

Agents were classified into four subgroups: targeted agent alone versus placebo or best supportive care (subgroup A); targeted agent alone versus nontargeted systemic anticancer therapy (subgroup B); targeted agent combined with nontargeted systemic anticancer therapy versus nontargeted systemic anticancer therapy alone (subgroup C); and chemotherapeutic agents (subgroup D). We compared the rates of treatment-related death, treatment discontinuation, and grade 3/4 AEs in each of these subgroups. In a separate analysis, we pooled the odds of hematologic and nonhematologic toxicities in each of the subgroups.

Overall, the finding of this subgroup analysis for the three endpoints of the study was described in the discussion thusly by the authors:

Our analysis shows that most newly approved anticancer drugs are associated with increased odds of toxic death, treatment discontinuation, and severe AEs compared with the standard treatment received by controls. There was an overall increase in the odds of toxic death, treatment discontinuation, and grade 3/4 AEs with the use of new agents versus the control groups except when targeted agents alone were compared with chemotherapy in the control arm. When individual toxicities were evaluated independently, the odds were increased to a greater extent for nonhematologic than for hematologic toxicities with the use of experimental agents compared with the controls.

In other words, when these new agents go head-to-head with existing cancer therapies, the OR for serious AEs is not elevated in the experimental group, while in groups for which the new agent was either tested versus placebo or best supportive care or combined with nontargeted anticancer therapies or with cancer chemotherapy the addition of the new drugs increased the chance of significant toxicity. This is about as expected a result as I can think of. Of course a new agent is likely to produce more toxicity than placebo; that is, if it?s an active drug. Of course adding a new agent to existing agents, be they traditional chemotherapy or other nontargeted agents will increase toxicity! It?s a general principle that an active drug will be more toxic than placebo and that adding another drug to an existing mix of drugs is likely to produce more toxicity! In fact, I?d be surprised if the investigators didn?t find that.

That?s the message that?s being lost in how this particular study is being reported. Now don?t get me wrong here. This study, admittedly, is disappointing. However, it?s disappointing not because it shows that the newer drugs are so much ?more toxic,? but rather because it suggests that, contrary to the hope and the hype, the newer drugs are probably not less toxic than existing drugs. Actually, the study doesn?t exactly show that. The numbers of studies looking at head-to-head comparisons of new drugs with established treatment regimens was too small. It does, however, strongly suggest that newer agents at the very least are not that much less toxic than existing drugs and might be more. It also confirms the long-known dictum in pharmacology that the more drugs you give at the same time the higher the chance of interactions and toxicity. Newer, ?targeted? drugs are not immune to that basic principle of pharmacology.

Another finding that seems surprising on the surface but really isn?t is that the observed toxicity of newly approved drugs did not correlate with how much they were able to increase OS or PFS. There is a misconception common even among doctors that the price we pay for more effective anticancer drugs is more toxicity, but that?s not necessarily true, and this study is simply more evidence to that effect. Another implication of this observation is simply to reinforce the point that we have to weigh toxicities very carefully versus incremental improvements in OS and PFS that result from adding new drugs to our cancer treatment regimens. There often comes a point where patients judge that the additional toxicity is not worth it for the amount of potential benefit that such drugs provide. In any case, it should not be surprising that toxicity and efficacy are largely independent properties of such drugs. As the authors themselves point out, efficacy is determined by the sensistivity of tumor cells and their microenvironment to the drug, while toxicity is more dependent on drug metabolism and clearance, as well as comorbidities, organ function, and concurrent medications, among other things.

What needs to be remembered about the new generation of more targeted therapies is that, unless they are used in patients whose tumors have the intended molecular target and are thus likely to be sensitive to the medication, the chances of benefit decline while the chances of toxicity increase. That is why, for instance, we do not give Herceptin to patients whose tumors do not make HER2 and do not use Tamoxifen or aromatase inhibitors in patients whose breast cancer is ER(-). For us to realize the maximum benefit at the lowest cost in terms of toxicity from these new medications, in addition to biomarkers that predict response (such as the aforementioned ER and HER2 markers) but biomarkers that also predict toxicity.

Finally, it?s important to note that, as incredible and copious as it is, all this genomic data doesn?t help us that much?yet. The reason is that we don?t yet know what to do with this ?firehose? of a data stream, how to apply the data contained therein to guiding patient care in order to cure more patients and prolong the survival more of those who cannot be cured. I actually believe that one day we will have the tools and knowledge to do that, such that I envision a day when we will use tumor biopsies and blood to construct comprehensive genomic, transcriptomic, and proteomic profiles of individual patient tumors and use them to target the molecular derangements that drive them. However, I am under no illusion that reaching that day will be easy, cheap, or without other cost. Studies like Niraula et al keep us humble.

The economy's growth slowed in the second quarter, government data showed, as consumers, worried about jobs and wages, held back.

The economy is stumbling and there is little evidence that leaders in Washington are willing to act to get it moving smoothly again.

Fresh data confirmed Friday that the growth of gross domestic product slowed sharply in the past nine months.?The Commerce Department?s initial estimate pegged second-quarter growth?at a 1.5 percent annual rate, the weakest showing since the third quarter of last year.

The slowdown was brought into sharper relief following the government's?revision Friday to estimates for prior quarters. First-quarter growth was bumped up by a tenth percentage point to 2.0 percent and output for the fourth quarter was raised to a 4.1 percent rate from 3.0 percent.

The report prompted the White House to cut its growth forecast for 2012 and 2013. In its semi-annual budget review, the White House said it now expects gross domestic product to rise 2.3 percent this year and 2.7 percent again next year ? down from the 2.7 percent and 3.0 percent growth projections it made in February.

In June, the Federal Reserve lowered its prediction for growth in 2012 to 2.4 percent, a half percentage point weaker than its previous forecast in April.

The latest data show the biggest drag on growth is coming from consumer spending, which accounts for about 70 percent of economic activity. After a spending perked up last year , driven by sales of cars and other long-lasting goods, spending grew by just ?1.5 percent rate, down from the 2.4 percent pace in the last quarter of 2011. That was the weakest showing in a year.?

A separate report Friday showed consumer sentiment tanked in July, falling to its lowest level of the year so far.

Consumers have reason to hunker down, following years of stagnant wages. Now, with the jobless rate stuck at painfully high levels, there is little news from Washington to boost confidence. As the economy continues to sputter, the Federal Reserve remains on the sidelines and Congress and the White House are stubbornly deadlocked over proposals to boost growth.

Hanging over all of this is the election in November. The economy's poor showing throws?a hurdle in front of President Barack Obama's campaign to keep his job. His Republican rival Mitt Romney, a former businessman turned politician, has made his financial acumen a central theme of his campaign.

The poor showing in the second quarter has raised hopes that the Fed will step in with another round of asset purchases known as quantitative easing. But most economists feel if the Fed acts at all, it won't be at its regularly scheduled meeting next week.

"This (economic data) is the sign that policymakers must act to provide more support to the economy if they want it to grow fast enough to start putting sustained downward pressure on today's still too-high unemployment rate," said Josh Bivens, research and policy director at the Economic Policy Institute.

That action doesn?t appear likely in the near term, especially on the fiscal front.

In the latest act in the ongoing congressional dog and pony show on the budget,?House Republicans agreed to a vote next week on a Senate-approved measure that would extend Bush-era tax cuts for households earning less than $250,000.

"If our Democrat colleagues want to offer the president's plan or the Senate Democrats' plan, we're more than happy to give them a vote," House Speaker John Boehner said Thursday.

But the Democratic bill almost certainly?will?fail in the Republican-controlled House, serving only to confirm that both sides are deeply deadlocked over the looming year-end tax increases and spending cuts put in place when Congress deadlocked a year ago over raising the government's legal borrowing limit.

Just as that standoff lasted until the nation was on the verge of a debt default, both sides are apparently committed to driving the government to the edge of a ruinous ?fiscal cliff? before acting. In the closing days of its current session, Congress will have to resolve the issue of expiring tax cuts and ?automatic? spending cuts that most economists believe would throw the U.S. economy into recession in a matter of months.

Spending cuts alone would result ?in an estimated 2 million lost jobs?next year?even as?the pace of hiring is already too slow to keep up with population growth.

With Congress and the White House deadlocked in partisan paralysis, the Fed is the economy?s last, best hope. With growth falling, speculation is rising over whether --?and when --?the central bank will begin pumping more cash into the financial system to spur more borrowing and spending by businesses and consumers.

'The Fed's concern and mandate is employment,? said Joseph Trevisani, chief market strategist at Worldwide Markets. ?Annualized GDP growth at 1.5 percent cannot begin to mend the unemployment picture. Mr. Bernanke and company have all the rationale they need to open the liquidity spigot.?

But Fed watchers say it?s unlikely that policy makers will announce any new moves to prod growth at next week?s regularly scheduled, two-day meeting. One reason is that?the latest data seem to bolster hopes that the economy may still dodge an outright recession. Friday?s GDP revisions, which included growth rates all the way back to 2009, tend to bolster that view.

The new figures show that the 2007 recession was not was deep as originally reported and that the recovery was not as strong. ??

Based on a review of additional data, including IRS tax returns, the government now believes gross domestic product grew by just 2.5 percent in the 12 months after the recession ended in June 2009, not the 3.3 percent rebound previously reported.

That means that since the recession ended, the economy has been growing at an average pace of 2.2 percent. In that light, the latest reading of 1.5 percent growth represents a somewhat milder slowdown from the longer-term trend. ?

Still, any reading of less than a 3 percent growth rate will be painful, because?anything less is too slow to bring down the unemployment rate. The economy needs to grow at a 2.0 to 2.5 percent pace just to keep the?unemployment rate stable.??Most Fed watchers believe that if sluggish growth persists, or worsens, the central bank likely will fire up its money pumps by the end of the year.

It remains to be seen, though, whether those moves will have much impact.

?Today?s (GDP) report may be enough to tip the balance of probability towards further monetary accommodation before the end of the year,? said Chris Jones, and economist at TD Economics. ?But will it give the real economy the boost it needs?? Probably not.?

Fed Chairman Ben Bernanke recently conceded that the Fed's powers to revive growth are limited without help from Congress and the White House, which seems unlikely in an election year.

To explain the quality theory of inflation we see it as founded upon the belief that a currency being used is acceptable and capable of being traded for goods and services, but also is beneficial to the buyer. However explaining how the quantity theory of inflation works is associated to the aspect of currency. This theory actually takes into account the currency's supply and demand aspects as well as its nominal value of exchange.

Inflation will create many effects upon an economy both good and bad. Inflation can target an entire society or even pin point its effects towards each industry, business or individual. The reason this is possible is because inflation itself is never evenly spread throughout society thus creating both its positive and negative effects.

Positive Effects

? Business Growth - inflation that is capable of being controlled, in that savings quite often can be invested and show growth. Opposed to where savings if held in a bank would show up as a net loss.

? Declining Debt Values - the true value of a currency will decrease as it becomes consumed by inflation. This translates to a decrease of real value on debt which would actually benefit those governments, businesses and private investors that were burdened with high amounts of debt effectively wiping out the debt completely in time.

? Increasing Stock Values - stocks previously purchased cheaper very well might increase in value and if timed correctly when sold off, would yield substantial returns.

? Increasing Asset Values - that are fixed may well increase which in-turn would make some businesses financially more stable. In a traditional sense higher prices often are a by-product of increased inflation. So in theory fixed assets would increase in value during times of high inflation.

Negative Effects

? Many costs are concealed towards reducing purchasing power. These hidden costs could prove an increased benefit to some, however harmful to many others as a whole.

? Soaring rates of inflation become harmful to the economy as a whole.

? Additional shortfalls within the markets would make it much more difficult for businesses to re- strategize, plan, budget and remain competitive for the long-term.

? Inflation could also emerge as interference for productivity, due to resources being moved away from businesses and their products and services while attempting to recover and regain profits.

? Rising inflation will additionally force labor to ask for increased salaries just to stay current with rising market conditions and higher prices on food, services and other living expenses. This ultimately will increase stress on businesses. Forcing them to boost net profits, lay off workers and increase prices for the products they manufacture. The result will only intensify the chaos on-going throughout the economy.

A small amount of inflation is always good for a society as it aids in competitiveness and more profits. An inflation rate of two to three percent is healthy but when inflation starts going out of control and worse we see runaway inflation with rates increasing at 10% per month we set the stage for hyperinflation. An economy suffering from hyperinflation will change an entire society into desperate times quickly. Sadly the United States and other nations are heading into this direction now.
